  • The medical certificate of death finding
  • The ID of deceased
  • The ID of the declarant
  • Birth certificate
  • Service record of the deceased (if it was male and shall not exceed the age of 55 years)
  • Registration is made within 3 days of death, otherwise, the prosecutor is required
